As per German tech blog Borncity, the file scanning issues emerged after Microsoft released an update to the Microsoft Malware Protection Engine (MsMpEng.exe) on September 16 that brought it up to version 4.18.1908.7.

Windows Defender version 4.18.1908.7 (KB4052623) apparently introduced this bug when Microsoft was trying to fix another issue that was brought through the July 2019 Patch Tuesday. The problem isn't specific to Windows 10, as other Windows operating systems also appear to be affected by this bug.
